As a nurse working through agency that previously worked at this facility full time roughly 12 years ago and was so excited to pick up shifts here, I absolutely hate to leave this review. However, I feel obligated to share my experience to protect other staff and residents.
I arrived for my first shift and found severe issues, including black mold under a sink (documented with photos), which I reported. Their only response was to remove a blanket that was placed under the mold, the mold itself remains untreated and poses serious health risks to staff and residents.
The workload is overwhelming, and there is no clear communication system in place. Another agency nurse and I had to create our own report sheet because the facility did not have a proper reporting or handoff system in place which is a basic safety standard in any healthcare setting.
Additionally, we discovered narcotic cards "secured" with band-aids on the back to hold pills in place which is shocking and unsafe practice that could easily result in medication errors or diversion.
I was scheduled for multiple shifts at this facility and, unfortunately, I did oversleep by two hours for my last scheduled shift. As soon as I woke up, I immediately got dressed and drove straight to the facility to fulfill my responsibility. (Was at the facility 6 mins after opening my eyes.) Upon arrival, I called the ADON (Cedee in which remained kind and professional upon all encounters) and was told they had already arranged on-call coverage and did not need me to work the remainder of the shift. Despite my effort to show up and communicate, they reported me as a no-call, no-show, causing my agency account to be DEACTIVATED, meaning I can no longer work through that agency at ANY facility. I was also blocked from this facility on the app and I am completely okay with that as I did not anticipate accepting any additional shifts past what I had already agreed to work. I then returned to the facility the very next day in person to clarify and resolve this directly with the Director of Nursing, whom I had also received a message from in the agency app referring to being "sketchy." When I politely asked the Director of Nursing (Crystal Harber) if she had a minute to speak with me, she asked my name and immediately said, "No, I’m not interested in doing that. We have already blocked you and everything." This dismissive approach shows a complete lack of professionalism and accountability from leadership.
I value my responsibility as a nurse and took accountability for being late; however, I also took every possible step to correct it and to communicate directly and transparently. The facility leadership’s unwillingness to discuss or resolve the situation, paired with serious safety concerns, makes this facility dangerous for both staff and residents. The workload is horrendous and the staff nurses are miserable and grossly underpaid.
I do not recommend working here or placing loved ones here until these major issues are fully addressed and corrected.
(I am happy to discuss this matter with any party from the corporate office as I feel certain I can shed light on a lot.) As for Crystal Harber, keep your same energy and do NOT attempt to contact me now that I have left this review, remember you aren't interested in talking to me. But I do feel compelled to leave you with a few words as I know you'll be interested in reading this review: From a former DON- YOU ALWAYS MAKE TIME TO SPEAK WITH ANYONE THAT ASKS TO SPEAK WITH YOU, especially a nurse. (Even if you would have asked me to come back tomorrow, that would have been completely fine with me.) Effective communication is considered a vital skill for a successful DON. As a DON you must be able to communicate effectively and openly with nurses to ensure policies and procedures are understood and to foster a positive work environment. Ignoring staff concerns and refusing to communicate openly is "bad practice" in case you aren't aware. Sometimes you just have to bite your tongue and listen, even when you aren't interested.

My dad was here first on hospice and had excellent care. Now my Uncle who is like a brother to me is there for rehab and wound care and he is also getting great care. In fact, I'm so impressed, I'm thinking of applying to work there myself. I believe in giving great care and I like to work with other people who feel the same. All places can have their problems if they need more staff. I have been pleasantly surprised at the employees here. They have really gone the distance to deliver great care. That is love in action. I appreciate them. No one likes to have to be in a nursing home, but the least we can do is give them the best care we can.

My aunt has been here for several years and, for the most part, loves it and the staff. She is mentally sharp and the staff love to visit and chat with her. You can always find things wrong with anything, but we feel she is safe and loved. In pre-covid days, her sister visits often.

My mother in law was admitted at 3 pm and we took her home at 6:30 the same day. She called and said it was horrible there. We drove to see if it was true and we arrive to find these observations.
It was unbearably hot, at least 85 degrees. Filthy walls, floors and furniture. I witnessed an elderly man in pajamas going through the dinner plates that were in the hallway for other patients. I heard a worker come up to a nurse and say " I don't know what's wrong with her but Mrs so n so is shaking really bad. He said gimme a minute and I never saw him check on her.
No way would I keep my family there!!!
